What belongs in an insider list amendment note

An insider list that only shows who is on it today is of limited use when a dealing six months ago is questioned. Amendment history — who was added, when, for which matter, and when they were removed — is what a filings review actually tests.

Minimum fields that survive scrutiny

• Date and time of addition or removal
• Name and company role
• Project or information label (even if coded)
• Reason for inclusion
• Who authorised the change

Common gaps we see in Leeds packs

Lists exported without amendment history; project codes that changed mid-deal without a bridge note; contractors added to email threads but never to the list. None of these gaps prove improper dealing. They do force the findings note to say the record is incomplete.
